A Photo Release form is necessary to protect photo agency/photographer if someone claims that photo agency/photographer did not have a proper permission of the person for using his/her likeness. This is a simple template of a photo release form to secure permission for use of an individual’s likeness with or without identification.

I hereby grant the Photographer/Agency permission to use my likeness in photograph(s) in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by the the Photographer/Agency, in perpetuity, and for other use by the the the Photographer/Agency. I will make no monetary or other claim against the the Photographer/Agency for the use of the photograph(s).
